What is a superbill?
Moonbow Mental Health files claims on behalf of those insured by most Blue Cross Blue Shield and Aetna plans and will collect any required copay for services. For other insurance companies or Blue Cross Blue Shield/Aetna plans that are not covered, a superbill is needed to allow clients to file claims. Superbills are sent to all clients who have elected to receive them on their Financial Agreement form completed upon intake at Moonbow Mental Health. After your session a superbill is generated and sent to you through your client portal which can be accessed here. The superbill serves as proof of your services and includes diagnosis codes, procedure codes, and all the other numbers insurance companies require in order to pay claims. Your insurance company will not reimburse for your sessions without this information.
How do I determine my out-of-network benefits?
Contact your insurance company at the customer service number (usually found on the back of your insurance card) and follow the steps below.
When you contact customer service let them know you are calling to determine your insurance plan’s mental health coverage. Tell them Moonbow Mental Health is an outpatient, out-of-network provider with a licensed clinical mental health counselor (LCMHC).
Ask the following questions:
Will my insurance pay out-of-network benefits?
How much will they cover for “out-of-network mental health office visits”?*
Is there a limited number of visits per year? How many?
Is there a deductible? How much?
Has any of the deductible been met? How much?
Is this deductible for a calendar year or other time period? What is the time period?
Do these services require authorization?
If yes, who needs to obtain the authorization, you or Moonbow Mental Health?
* It can be helpful to have the Service codes when getting pre-approval or to ask if there are different coverage amounts for these different services. These are the most common CPT codes used at Moonbow Mental Health: 1. Intake & Assessment Sessions: 90791 and 2. Psychotherapy 60 min. 90837
How do I submit my superbill?
For those with Cigna, United Health Care, or Blue Cross Blue Shield of NC Plans please find your designated claim form below. If you have other insurance, you will need to go to your insurance company account online and print out a blank claims form and complete it.
Attach the superbill(s) to the completed form. Be sure to state on the form that we are out of network and that you have already paid us for the appointment so that the reimbursement will come directly to you. Look to see if there is a place to mark yes or no to “accept assignment.” Mark your answer “no” so that they pay you rather than Moonbow Mental Health.
Remember: the sooner you submit your superbill, the sooner you will receive reimbursement.
